Request for Name Records Check (CANBY ONLY)

Name Checks are performed only for the person making the request - photo identification is required upon submission.  This inquiry and any response pertains only to records of the Canby Police Department and does not include any other jurisdictions / law enforcement agency records. 

If you need to receive a copy of your own Oregon Criminal History report or a Clearance Letter stating that you have no Oregon criminal history, you must contact the Oregon State Police to make a request.

A fee of $10 is required at the time of request for a Name Check.  Once your payment has been received, the response will be prepared.  This may take up to an additional 5-10 business days.  The written response will be emailed unless other arrangements are made.  Payment can be made in-person at the Canby Police Department during regular business hours, Monday thru Friday, 8am to 5pm. Payments can also be taken over the phone using a credit / debit card by calling the Record Department at (503) 266-1104 ext.0, during regular business hours.
